Individual Service Agreements (ISA) and Person-Centered Plans (PCP) are developed with each person supported and serve as a roadmap for the supports provided. The individual (or their designate) leads in creating these plans to ensure they meet the person’s needs.

Person-Centered Plans are the heart of everything we do – it’s an opportunity to celebrate the person supported, their story, and find out what is meaningful to them. Family members, friends, staff, and community members are invited to participate, and discussions with the supported person help to determine meaningful activities and identify personal goals – dreaming big and thinking outside the box is encouraged!

Since these are personalized plans, they are reviewed annually and adjusted as the individual’s needs or choices change. An Individual Care Coordinator assists the person supported in creating and revising the plan along the way. For example, the Individual Care Coordinator may assist the person in accessing their needs in the following areas:

  • Education and employment
  • Health professionals
  • Access and transportation
  • Generic and specialized services
  • Home and personal care supports/guidance
  • Community and social activities
  • Communications
  • Finances
  • Family
  • Recreation

Within each area of support, a variety of support options can be provided. For example, in the area of transportation, the Individual Care Coordinator can arrange out-of-town travel; make calls, appointments and arrange bookings, assist with applications and obtaining travel cards or teach individuals how to make their own arrangements.
