PHSS realizes people with disabilities have special needs when it comes to their home environment. At PHSS, we make each house accessible for those who live there. This means some homes need ramps, ceiling lifts, raised bathtubs, and walk-in showers to ensure they are safe and comfortable while others require less extensive modifications.

A home supporting individuals in wheelchairs requires some changes to make life as accessible and independent as possible for the residents. These changes can include modifying the kitchen sink height so it can be reached from a sitting position or making the hallways wide enough to accommodate an electric wheelchair.

Our support workers use assistive equipment to help people move from one place to another. Portable lifts make it possible for people to move from their wheelchair to a couch or comfortable reclining chair.

Project Hope “Help with Opportunities for Participation and Enrichment” is a full-day program for young adults who are living at home with their families, who have recently graduated from high school and who because of their complex/high needs are unable to access existing community programs/supports or services.

Service Descriptions:

Supportive Housing – Assisted Living (complex/medically fragile)

PHSS supports individuals with developmental disabilities and/or complex physical needs to live in their own homes, participate in community and enjoy life with family and friends.

PHSS offers supports in homes throughout Ontario by highly qualified staff.

Specialized programs are available for persons with Acquired Brain Injury (ABI), complex medical, children’s services, behavioural supports and supports for persons on Long Term Mechanical Ventilation Program

24 hour individualized supported are provided which include:

  • Access to Health Professionals
  • Transportation
  • Communication
  • Recreation & Leisure
  • Education and Employment
  • Community and Social Activities

Day Respite

Flexible centre-based program options for adults with complex physical needs who are no longer eligible for educational support, who live at home with their families and who are totally dependent on others for their care, safety and well-being. Overnight Respite

Available to persons over 18 years of age who reside in the City of London, and Middlesex, Huron Perth, Oxford, and Elgin Counties who are considered medically fragile and have needs defined as “complex care”. Examples include, but not limited to, ventilators, enteral nutrition support (G or J Tube) and continuous oxygen.

Family Resource / Passport Funding

This community based staff resource is available to individuals with disabilities and their families with individualized and/or direct funding (from the Ministry of Children, Community and Social Services and/or the Ministry of Health and/or private sources), families waiting for service and to family groups.

Support and assistance can be time-limited to address a specific need or request or it can be flexible and on-going. It involves direct support as well as planning and facilitation to assist families to manage their individualized funding. The FRC can create individualized job postings to family’s specifications (days and hours of work, focus of work, requirements for the job etc…) and circulate internally to staff and on our web-site, distribute to other agencies, post on Fanshawe web-site, free of charge. Also available to assist and support families with interviewing and creating contracts between family and worker, between family and agency to clarify roles and responsibilities of each. Provide support to workers during initial stages and on-going support to find meaningful activities in the community.
