Pathways to Support
The purpose of this page is to help individuals, families and caregivers understand how to access supports and services at PHSS.
PHSS does not manage it's own waitlist or intake. We are only able to provide supports and services to individuals through the DSO and Health atHome waitlists.
Note: This does not include our fee-for-service program for which we can intake directly (below).
Start with the provincial pathway that matches your needs.
Funding comes through MCCSS (Ministry of Children, Community and Social Services) and the Ministry of Health. The application process depends on the type of supports and services needed.
These agencies assess eligibility, manage waitlists, and ,may match or refer you to PHSS (Hint: agency coordinators may refer to PHSS as "Participation House London" in their systems).
You can express interest in receiving supports with PHSS; however, neither Development Services Ontario or Ontario Health atHome can make placement promises.
