Pathways to Support
The purpose of this page is to help individuals, families and caregivers understand how to access supports and services at PHSS.
PHSS does not manage our own waitlist or intake. We are only able to provide supports and services to individuals through the Development Services Ontario (DSO) and Ontario Health atHome (OHAH) waitlists.
Note: This does not include our fee-for-service day supports at Community Place North, which we can intake directly (see online inquiry form below).

PHSS is funded through Ministry of Children, Community and Social Services (MCCSS) and the Ministry of Health. The application process depends on the type of supports and services needed as noted in the above flow chart.
DSO and OHAH assess eligibility, manage waitlists, and may match or refer you to PHSS. Agency coordinators may refer to PHSS as "Participation House London" in their systems.
You can express interest in receiving supports with PHSS; however, neither Development Services Ontario or Ontario Health atHome can make placement promises.
