Our Journey: Engaging
Community

From our roots to today, discover how PHSS has championed inclusion, independence, and living well in community.

Founded in 1988 with a vision for healthy communities where everyone belongs

In 1988, a small group of families and advocates came together with one clear goal: to create opportunities for people with developmental and physical disabilities to live close to their loved ones, not in institutions.

From the very beginning, PHSS believed that everyone deserves the dignity of choice, the comfort of home, and the opportunity to participate fully in their community.

What started as a grassroots initiative quickly grew into a trusted non-profit, recognized for its compassionate and innovative supports.

Fresh Identity - Same Guiding Purpose

As PHSS grew, so did the breadth of its services. By 2016, the organization had evolved far beyond its early days, supporting individuals with increasingly complex medical and developmental needs.

To reflect this growth, PHSS refreshed its identity with a modernized name, new branding, and renewed focus on its mission. The rebranding wasn’t just a new look, it was a promise: to stay flexible, forward-thinking, and deeply rooted in the values of inclusion and community living.
