PHSS, in partnership with local hospitals, Ontario Health at Home, Community Respiratory therapists, and other healthcare partners facilitates eight protected beds allocated to those who are long-term ventilator dependent (LTV) and require specialized support in order to remain living in communities. This partnership’s success allows for people, who were living in Intensive Care Units (ICU), to move into supported homes in the community.
Moving care and support into the community not only reduces strain on hospital resources but also ensures a person-centred approach where each person’s preferences, routines, and social connections are valued.
People supported receive care and support from trained staff with expertise in complex medical needs, respiratory care, and person-centred care and support. In addition to residential services, PHSS provides supplementary support during hospital stays, ensuring continuity of care and familiar support during transitions.
Homes are fully accessible, inside and out, and designed to adapt to each person’s unique needs. Accessible transportation, opportunities for community involvement, and access to day supports help people supported participate fully in their community.
